Please review the terms & conditions of our general waiver, photo release, and payment authorization prior to signing up for your membership.
Payment Authorization
Monthly dues for membership will be automatically withdrawn the same day of every month unless otherwise noted and a 30-day notification is required upon cancellation of services via Enzo Athletic Performance’s Exit Survey Process.
All clients must provide ACH or credit/debit card number below. In the event Enzo Athletic Performance is unable to collect payment electronically, the member will be notified and payment must be made immediately to continue participating in any program at Enzo Athletic Performance.
Release, Indemnity, Agreement and Covenant Not To Sue
Clients agree to forever refrain from instituting, prosecuting or in any way aiding any suit or claim against Enzo Athletic Performance and its members, individually, its successors, and assigns, and all other persons, forms or corporations for whose acts to whom they may be liable for any injury or death resulting, or to result from negligence or other acts on the part of the client.
Clients hereby understand and agree that utilizing exercise services and equipment may be dangerous, and are fully aware of the risks and hazards involved.
Clients understand that it is their responsibility to consult with a physician prior to, and regarding utilizing exercise services and equipment.
Clients represent and warrant that they are physically fit and have no medical condition which would prevent me from utilizing exercise services and equipment safely.
Clients, in consideration of being permitted to utilize exercise services and equipment, agree to assume full responsibility for any risks, injuries, or damages, known or unknown, which might incur as a result of participating in said utilization of exercise services and equipment and will indemnify and hold harmless Enzo Athletic Performance and all parties associated from any and every claim or demand of any kind or character which may be asserted by any person, corporation, or entity on my behalf.

Clients agree that Enzo Athletic Performance and all associated parties are NOT responsible for the supervision of any children, furthermore, that all parents are responsible for the safety and supervision of their own children.

Clients will rely wholly upon their own judgment, belief and knowledge, and no representations or statements regarding this matter or any other matters made by, or on behalf of the Enzo Athletic Performance and all associated parties (who is to be hereby protected) has been relied upon by the undersigned to any extent whatsoever in making this agreement.

Use of any picture(s)/film/social media/likeness: Clients agree to allow Enzo Athletic Performance, its trainers, owners, and bodies associated to use pictures, films, and the like for advertising purposes.
